The opioid crisis has led to record overdose deaths, with a rate of a lethal opioid overdose every 11 minutes in the US. Over the past 20 years the crisis has evolved from prescription opioid exposure, to increasing heroin use, to most recently high-potency synthetic opioids. We will discuss opioid use trends, history and regulations, neurobiology, intoxication and withdrawal, and medication pharmacology. We will focus on clinical decision making to support utilization of medications and review available formulations.
