205 - Getting It Right Every Time, Standardizing Specimen Handoffs
Description: Specimens were being mislabeled in the operating room, there had been seven mislabels in three months. This has many implications for the patient, the team, the doctor, and the hospital. This had to be stopped. A performance improvement project was created using the department manager, assistant manager, and evening charge nurse as the team. A literature review was completed and it was decided to standardize the moment of specimen hand-off between the circulator and the scrub tech. Education was created and given to all staff, including the surgeons. Audits of hand-offs were done. The outcome has been zero mislabeled specimens in 12 months and counting. By standardizing specimen label hand-off, patient safety is ensured and specimens, which are needed for proper diagnosis and treatment, are attributed to the correct patient.
