Description: AbstractAs we know, drainage tube application after surgical operation may cause infection,which is followed by a high rate of death and disability , re-draining risk and hospital staying period. We have designed a new easily-made multi-function splicer ,made up with drug resistant three-way valve and conical connectors, it can be used to adjust the volume of flushing speed control flushing, avoid to take off the tube drainage device. This splicer was used in 32 patients without infection. We assured that it can reduce the workload and operation time, so that it improve the safety of patients and operation efficiency.BackgroundIn surgical operation drains are usually used, as one of its application——extra ventricular drainage is an effective treatment for hydrocephalus, cerebral hemorrhage, intracranial hypertension and central nervous system infection, while drainage tube application after surgery may cause intracranial infection by rate of 2%-27% according to articles, besides ,infection usually followed by a high rate of death and disability in addition to re-draining risk and hospital staying period. Whereas there are defects on the current draining operation, one is absence of control valve which makes it hard to conduct flushing operation, another is necessity of replacing the drainage bag with injector when flushing, in this way it is more likely to be infected.Clinical IssueAt present, flushing is operated in this way, first to pull the drainage tube open by forceps for pushing in the tee end, at this time it requires mersilk frapping operation to be applied due to mismatching of connecting device.Dfects on current connecting methodFirst: being complex, time consuming and apt to be loose.Second: drainage tube might be damaged during operation and followed with risk of leaking infection. When tee’s connecting end with size of 3.81mm confronting 2.0-2.6mm of the tube hardly it will not burst and cause leaking.Third: high cost and risk. There are imported devices available on the market with price around 3000 yuan per set of which patient be seldom affordable. Besides, those imported drainage tubes are of limited sizes, being not suitable for all of the patients. Fourth: need longer operation time and more nursing work involved.Description of TeamThe team is guided by experienced specialized nurses, with the cooperation of specialized anesthesiologists and specialized doctors. The nursing staff have rich clinical experience. The members have 2 senior professional titles, 5 intermediate professional titles, 1 doctoral student and 4 master. All of them have rich clinical experience.Preparation and PlanningWe hope that it can develop a new connecting device free of flushing, sampling and complex operating issues, to drop down risk of infection, offer a more reliable operation, avoid re-draining and relieve lots of medical work.The draft of the multifunctional drainage tube connecting device has been designed, and has been granted the national utility model patent right, patent number: 201621183278.8 (see research foundation), and has successfully applied for the center's new technology and new project. Fifth: avoiding tube damage, reducing risk of infection and re-draining.Result/OutcomeThe connecting device has been used on 32 cases with no infection occurring, all the patient involved were discharged in good condition after decannulation.The connection time of nurses decreased from 20min to 5 s, and the extubation rate and leakage rate decreased significantly after operation. Nurse satisfaction improved. The patent has been applied and won the Innovation Excellence Award of Hong Kong Innovation Competition Innovation Excellence Competition ,the Third Prize of Nursing Instruments Innovation of the National Pediatric Nursing Union and the Second Prize of Guangzhou Innovation Competition,The results were read out at the annual meeting of Guangzhou Nursing Society.The patent has been applied . Implications for Perioperative NursingIn comparison with current drainage tube and tee that need silk frapping our device has advantages of easier operation, better connection, non loosening and free of frapping, making patient more comfortable and flexible, avoiding issues of drainage tube , reducing risk of infection and re-draining. The device is suitable for surgery involving drainage tube flushing and sampling. The patent has been applied for it for present. If we establish cooperation with a factory and manufacture it in batch, a wide prospects will be get. From the patient's point of view, the innovative device can provide a more comfortable and safe postoperative experience and reduce medical costs. From the perspective of medical care, it can shorten the operation time, spend more time on patient care, and save manpower and material resources.The innovative device free of flushing, sampling and complex operating issues, to drop down risk of infection, offer a more reliable operation, avoid re-draining and relieve lots of medical work.The innovative device has brought new experiences to both patients and medical staff.
