110 - Applcation of Healthcare Failure Mode and Effect Analysis in the Safety Management of Intraoperative Radiotherapy
Description: [Abstract] Objective To explor the application value of health failure mode and effect analysis(HFMEA) in the safety management of intraoperative radiotherapy(IORT ) .Methods The HFMEA group was set up to analyze the potential risk of breast cancer patients undergoing intraoperative radiotherapy from January 2014 to April 2018. The group would list the failure modes that need to be improved,identify the failure step that need to be intervened, determine the causes of the failure modes and calculate the risk index (RPN).Then, made the safety management plans and put them into action. Finally,we would observed the implementation effects.Results The RPN score of the failure mode was significantly lower than before and lower than the critical value which need to intervent. The incidence of the adverse events was lower than before.The difference was statistically significant (P < 0.05). The absorptive scattering dose of different tissues was lower than before.The difference was statistically significant (P < 0.05).The satisfaction of the patients and doctors was higher than before,.The difference was statistically significant (P < 0.05).Conclusion HFMEA is an effective safety management method, which can effectively improve the safety of operation and ensure the safety of patients.
