286 - Caresense Digital Innovation Transforming Patient Engagement
Description: Ineffiencies can affect every domain of nursing ranging from preop assessment, patients experiences, operating room start and stop times, throughput and hospital readmission rates. A lack of standarization in perioperative practice can lead to ineffiencies.Digital innovation are interventions designed to increase standardization in clinical practice. CareSense is a form of digital technologly which can be used to send patients standarize educational information and preoperative and post operative reminders by way of email and text messages. Patients undergoing designated surgical procedures would recieve standarize text messages and educational information. Additionally these patients would be monitored the same way.Digital innovation technology can enhance a patient's understanding of the surgical procedure preoperative and postoperative. CareSense digital innovation improves patient satisfaction, decrease length of hospital and readmission rates. CareSense is an digital intervention currently being used in cardiac and the orthopedic service lines. A collabartive team which included 400 clinicians of all disciplines: nurses, surgeons, physicians, nutritionists, pharmacists, anesthesiologists, quality safety officers, blood conservationist and Epic team members develop the CareSense project.The writers consited of a core group of 10 people per each surgical service line (i.e., each care pathway) . Each pathway was circulated among at least 30 additional people to review the pathway. The start of the pathways in EPIC was implemented with EPIC team members assistance to train nurses, surgeons, hospital and office staff. The CareSense project began in April 2018. The goal was to write content for patients undergoing colorectal surgery, lumbar fusions, total joint replacements and coronary artery bypass procedures. Educational information was provided to patient's and family members on how to prepare for surgery, when to obtain clearances , when to start and or stop medications. Patients also were informed on how to enhance their recovery as well as what to watch for during recovery. Patients are provided with instructions such as "Take a look at your incision site" "Is the site red?"If the patient reponds "yes" the care team is alerted so the patient can receive additional monitoring or evaluation. The CareSense Project has been rolled out to cardiac and orthopedic service lines. Results and Outcomes. There are 1904 patients on Caresense. Some important outcomes include 92% of patients report that Care Sense improved their understanding prior to surgery. 93% of patients "actively" engage in CareSense. Results show decrease of patient phone calls.CareSense is now expanding pathways beyond 8 surgical procedures which will include 43 additional surgeries by the beginning of 2020. CareSense Digital Innovation is a valuable tool which increases standarization and reduces ineffiences of perioperative nursing care.
