Leaders in the SNF and long-term care profession are recognizing the opportunity to improve the sector’s approach to caring for patients discharged from hospitals and/or who are frail and in need of medical and social supports. In addressing the burdens on our emergent-care systems, the profession is discovering new ways to care for patients – whether through more on-site skilled nursing, COVID-dedicated care, or by more effective use of telehealth.
