Person-Centered Care (PCC) has become a buzzword in human services, but what does it really mean? Beyond federal regulations, person-centered care puts the individual in the driver seat of their care coordination. This session will describe the key components of PCC and how can they be accomplished for the population in long term care, including ID/DD, SNF and AL settings. This program will discuss the McCormack and McCance’s key components of person-centered care and offer practical application in areas of building staff mindfulness/resilience to improve engagement and awareness during resident interaction, and building a culture that promote shared power between staff, residents and families.
