What is Population Health? In this session we will define population health, as it is not going away. We'll also define ICNs, ISNPs, DSNPs, etc. We'll focus on what the impact of population health could be on your center, explore/broadly define models of care, facility incentives, and its impact on residents. Population health also involves return to hospital prevention/treat in place support, health risk assessments and more. How do I implement and support population health in my center? Facility Champion, know and understand your metrics, relationship management of population health providers (monthly meetings, metric review, daily stand up discussions, communication with NPs, etc.
