This presentation will provide important education about prevalence and impacts of social isolation and loneliness in the post COVID-19 era. The speaker recommends evidence-based community policy development with assessment measures and person-centered strategies for reducing resident social isolation and loneliness. The speaker will provide the audience with evidence-based assessment tools specifically to scale level of social isolation and loneliness and strategies for implementation for person centered care planning and coordination.
