Nearly 20 years ago, The Institute of Medicine published the groundbreaking report “To Err is Human: Building a Safer Health System” that estimated preventable adverse events are a leading cause of death in the United States. Medical institutions including ambulatory care settings have greatly improved patient safety by implementing systems to track, measure, and analyze patient safety events and near misses. Very little work has been done in dentistry. This seminar will demonstrate how dental institutions can effectively identify, measure and learn from patient safety events and near misses and the steps needed to establish a culture of patient safety.
Recongnize the importance of patient safety event reporting and the advantages of institutional learning and performance improvement.
Analyze how other institutions are tracking and learning from patient safety events.
Reflect on their own institution’s reporting and management of patient safety events and identify opportunities for improvement in practice and in the curriculum about patient safety.
