The value of care provided by traditional rheumatology practices is compromised by workforce shortages and inefficient delivery of care processes. The goal of this presentation is to share practice team and workflow innovations from the Rheumatoid Arthritis Practice Performance (RAPP) Project that have increased access to care for new and established patients, eliminated wait times for necessary care, and provided Treat-To-Target outcomes for many more patients per physician at a lower per patient cost. Utilizing Advanced Practice Clinicians to perform and document patient assessments and coordinate care to allow rheumatologists to focus on new and complex established patients' management are essential to achieving these practice improvements.
