This session is designed to provide the learner with the tools necessary to provide the best, safest care for their elderly patients. Learners will learn appropriate goals of care for their older adult patient with diabetes mellitus, pitfalls and appropriate use of diabetes pharmacotherapy, and practical solutions for avoiding common issues when these older adults transfer from one care setting to another. This interactive, case-based session will follow an older adult from independent, community-dwelling, ambulatory diabetes care through assisted living and finally long-term care. This session will also highlight issues seen across the spectrum of care as it relates to the care setting itself, as well as opportunities within care transitions.
This session is approved for Board Certified Ambulatory Care Pharmacist (BCACP) and Board Certified Geriatric Pharmacist (BCGP) recertification credit.
