The Northern Michigan Public Health Alliance (Alliance), a partnership of seven local health departments, embodies Public Health 3.0 and the role of Chief Health Strategist. Based on our experience in population health and clinical community linkage models, Michigan Department of Health and Human Services designated the Alliance as the backbone organization for the Northern Michigan Community Health Innovation Region, one of five “CHIRs” in the Michigan State Innovation Model—and the only one led by public health.
In this session, we will describe our approach to strategic leadership across the 31 counties in the Alliance; review our experience using the ABLe Change Framework with cross sector partners to address the social determinants of health impacting obesity and overweight; discuss a Health in All Policies initiative with private and public partners; and describe implementation of three Community Connections HUBs linking high needs clients to resources in the community using Pathways. In this session, participants will learn about the Alliance experience as Chief Health Strategists intended to build healthy and thriving communities across a large rural region.
