Conditions in the environments in which children, families and communities live have significant impact on their health and quality of life outcomes and risks. These conditions are particularly common and wide-ranging among families cared for in clinical community care settings. Local public health services and pediatric health care delivery should collaborate in identifying and addressing these issues.
Partnerships between pediatric providers and local public health agencies can play important role in safeguarding the health for children through a population-health system focused approach. By combining expertise and leveraging their respective strengths and shared vision for healthy communities, these partnerships are positioned to influence the health system.
This presentation highlights examples of how clinical-community partnerships, through the investments of the American Academy of Pediatrics (AAP) have worked to implement projects aimed at strengthening health systems and clinical services. A facilitated discussion with participants will identify skills and resources for local implementation of clinical-community partnerships.
