Public Health Madison & Dane County (PHMDC) has taken innovative steps to use their Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP) to center the voices of those most impacted by health disparities in their community. By shifting the decision-making and leadership power from the local health department to the community, the CHA/CHIP process can be used to understand community assets and challenges faced by its residents, and ultimately address the root causes of disparate health outcomes.
This presentation will provide public health professionals with the tools and information needed to more fully address the social determinants of health facing their communities through the CHA/CHIP. Participants will be able to take the lessons learned from PHMDC in facilitating community engagement, and apply them to their own communities. Participants will learn how to expand their role in advancing changes to the economic, social, and physical environment to promote health.
