Perinatal Health Partners (PHP) offers a comprehensive, integrated approach to perinatal care for women in 11 counties in rural south GA who are medically at high risk for adverse birth outcomes including maternal and infant mortality, low and very low birth weigh, or other medical or developmental problems. PHP has consistently been able to improve average gestational age births from 26–28 weeks to 37 weeks gestation. PHP provides intensive nursing home visits, including assessment, education related to the clients specific needs, and referrals in 11 rural south GA counties. PHP is in consistent communication with the client’s primary care provider to improve the health of the mother and unborn child.
