This presentation will present the results of a retrospective study of a set of community health resource centers established between 2003 and 2005 in a rural region of TX to increase access to care for rural residents. The case study documented the efforts of the cities, counties, researchers, staff members, and organizational partners for the establishment of the resource centers. This session will present details from the case study, providing an overview of the Community Health Development process as a planning model, a description of health resource centers from inception to evaluation to lessons learned, and illustrate the partnerships’ growth and change from the evaluation, with the intent to provide other communities with information and strategies to develop community health resource centers in their communities.
