The Promoting Health and Cost Control in States (PHACCS) initiative identifies and promotes effective, community-wide, non-clinical strategies that address the social determinants of health to improve population health outcomes. This presentation provides a forum for policymakers, public health champions, and professionals from other sectors to better understand what it takes to develop, enact, and implement multi-sector, policy-level population health policies that can effectively address the social determinants of health. The PHACCS initiative supports the adoption and effective implementation of a wide-range of policies in cities and states across the country.
