This presentation describes a quality improvement initiative for the Community Health Assessment and Improvement Planning process (CHA and CHIP) in the Florida Department of Health in St. Johns County (DOH-SJC). St. Johns County, FL has a population of approximately 243,000 and is currently the 14 fastest growing county in the United States. The journey began in 2005 with an initial Community Health Assessment and continues through the development of the 2017 CHA and 2018-2020 CHIP.
The presentation illustrates how the Director and Health Officer, Dr. Dawn Allicock, MD, exercised her role as Chief Health Strategist, and how workforce development and strong leadership resulted in improved workforce engagement, improved community engagement, and a great CHA and CHIP that today serve the community as a strategic plan for health. DOH-SJC has been recognized four times as a role model organization (2009,2011,2015,2017) by the Florida Sterling Council (State Baldrige Award) and is part of the PHAB accredited Florida Department of Health. The presenters are Ms. Noreen Nickola-Williams, Director to the Office of Public Health Practice and Policy, and Mr. David Klater, Quality Improvement and Malcolm Baldrige Award Consultant, both of whom are highly experienced practitioners of Strategic Planning and Community Health Improvement initiatives.
