Background: : Around 2.5 lakh – 7.5 lakh hospital code resuscitations are made annually in the Unites states. However survival rate is 0-59% with a mean of about 20%. The key factors leading to the failure is interaction with poorly designed system. The intention was to analyse the ergonomics related to the code event at our centre for improving the safe outcomes.
Research Question: : To study the human factors in Real time Code blue debriefing events with or without the help of Visual assisted device.
Methodology: : Retrospective data of code blue events from January 2018 – December 2018 were collected in our tertiary care Pediatric centre. Analysis of human factors were done by reviewing debriefing documents using Team Emergency Assessment Measure (TEAM) Scale. Analysis of video recording of the real time code blue debrief events were done wherever available. Data from the documented debriefing forms and from the video recording was compared and the results were analysed.
Results: : Total immediate survival rate in our centre after code blue analysis was 70.5%. Out of them ER cardiac arrests were 35.29% and In hospital arrest were 64.71%. It was found that, Team morale (97%), team compassion and control (91%) followed by leadership (88%) were found to be of good quality in our centre. Communication (69%), cooperation and coordination (75%) were the most common problems identified followed by situational awareness (80.7%) and adaptability (82.2%). When video recording was compared, it was found that difference was noted in terms of communication (25%) followed by cooperation and coordination (12.5%), adaptability (12.5%), situational awareness (12.5%) and prioritization (12.5%), but the P value was insignificant.
Discussion/Conclusions: : Team morale, compassion and control are of good quality in our centre. Clear communication, cooperation and coordination, situational awareness and adaptibilty were the identified lacunae in our debriefing session. Good quality video recording of the real code blue event might add more value. Even though the P value was insignificant, there is a trend towards video recording the real time events, will help not only for the analysis of participants but will also help the de-briefers to look upon those missing links which will help them to evaluate the participants. Further larger study is needed to understand the implications of using video recording to facilitate debriefing on a wider scale.
