Context: : PICU Hospital General San Juan de Dios / Universidad de San Carlos de Guatemala is the main facility in country. The teaching skills to develop adherence and use of SPIKES Protocol to delivery bad news in pediatrcis residents is challenge. The pediatrics and PICU postgraduate residents neeed a tool to have the possible approach to parents complex situation. We understand too even to Multidisciplinary postgraduate students in Guatemala at Universidad de San Carlos de Guatemala do not have an objective tool to develop the skills. The Project was developed in May 2018.
Description: : We define in each step as the SPIKES S = "Setting", P = "Perception", I = "Invitation", K = "Knowledge", E = "Empathising and Exploring",
S = "Strategy and Summary" a different scenarios based in several and most common situations to approach in cultural context in Guatemala. We introduce a condition about cross crultural, religious, socio.demographic, chronic conditions, acute notice to unexpected death and permanent damage. After define each scenerio we discuss a common conditions to define the educational objective, describe the idea to film director with whom develop the script and production. We develop the script and film schedule and after the perfromance of the scenarios recorded in video it was sent to post production. 7 videos, 1 hour aprox all of them are avallable in YouTube Channel UTIP HGSJD GUATEMALA
Observation/Evaluation: : After the developmet the Video Tutorial was released to 391 Medical Postgraduate students, broad as Internal Medicine, Surgery, Pediatrics and almost 11 more disciplines to discuss and develop a OSCE practice with Ethical Dilemma cases. All of them use as a tool to after the deliberation have a structure to delivery hard and bad notices to the families. All of the describes as useful tool and is important because is a reference to training freshmen in postgraduate students.
Discussion: : The approach to delvery bad noticies in post conflict society such as Guatemala; where we need improve the patient empowerment based in respect of cross cultural, religious, socio economical conditions. The Postgraduate medical students need a tool to recognize the most common scenarios based in SPIKES protocol, and the tutorial developed is useful in ethical dilemma cases
