Since 1984, various autogenous grafting procedures were developed, including minimal invasive approach (Carrot Technique) for small bony defect as well as lateral bone block grafting or 3D reconstruction for heavy and vertical bone defects to perfect implant placement in the mandible and the maxilla, improving function and esthetics. In the majority of the cases, the bone block graft is harvested from the mandible using the MicroSaw Technique. Following a precise protocol, this kind of harvesting procedure offers high security and allows getting bone blocks up to 4.4cm3. The techniques of augmentation with mandibular bone block were modified to increase the number of regenerated and vital osteocytes in the grafted area (biologic grafting) for a predictable long-term osseointegration. These techniques are based on the use of a combination of thin bone block grafts and particulate cancellous and cortical bone chips, which possess high revascularization and regeneration potential. These techniques are performed with pure autogenous materials without any membranes or biomaterials.
