Balancing tumor control and side-effects is one of the main challenges in radiotherapy. New research suggests that radiation damage to healthy tissue can be greatly reduced when the radiation is delivered extremely fast, i.e. whole treatments in milliseconds. Such extreme dose rate irradiation, also called FLASH therapy, can be achieved with existing proton therapy systems, however, the mechanism behind this effect is still unknown. The goal of this symposium is, to evaluate the current status of FLASH therapy and discuss the potential implementation in current and future proton therapy systems. Fast delivery of proton therapy could be used to shape the dose distribution and dose rate for each pencil in intensity modulated proton therapy (IMPT). However, the effects of FLASH therapy have mostly been demonstrated with electron beams. Depending on the underlying mechanism, proton FLASH therapy may not be feasible or ideal for clinical translation. We first discuss technical challenges in proton FLASH therapy, including how to achieve sufficient dose rates, ensure accurate online dosimetry and guarantee patient safety when entire treatment doses are delivered in milliseconds. However, clinical translation depends on strong in vitro and preclinical evidence for improved outcomes. Results of first extreme dose rate proton irradiations will be presented and the outlook for clinical proton FLASH therapy will be discussed.

Biography:
David J. Carlson, PhD, DABR, FAAPM is an Associate Professor at the Yale University School of Medicine and a Medical Physicist at Yale-New Haven Hospital. He completed his residency in Radiation Oncology Physics at Stanford University and received his PhD from Purdue University. He is a Senior Associate Editor for the International Journal of Radiation Oncology, Biology, and Physics, a member of several ASTRO committees, and a past member of the Board of Directors of the AAPM.
