In recent years, the Institute for Safe Medication Practices (ISMP) has highlighted a variety of errors that have negatively impacted intravenous drug therapy safety. These events are related to compounding and intravenous medication practices that occur within the pharmacy department as well as outside the pharmacy and involving different practitioners. The United States Pharmacopoeia is set to release an updated version of Chapter 797 Pharmaceutical Compounding – Sterile Preparations, and a new USP Chapter 800 Hazardous Drugs- Handling in Healthcare Settings. In addition to these regulations, ISMP has published Guidelines for Safe Compounding and Safe IV Push Practices to identify risk factors and point practitioners toward best and safer injection practices. It is essential that an infection preventionist be familiar with the key components of the regulations as well as Best Practices identified by ISMP.

• 1. Identify common unsafe practices/at-risk behaviors associated with the preparation and administration of injectable medications.
• 2. Recognize key components of USP 797 Pharmaceutical Compounding – Sterile Preparations, and USP Chapter 800 Hazardous Drugs- Handling in Healthcare Settings standards.
• 3. Describe challenges to safe preparation and injection practices.
