Efficient and effective management of large statewide programs to address known population risks and needs is a challenge for Title V across the country. MCH leaders in Tennessee recognized a need to streamline outreach and care coordination services to standardize practice and measure outcomes. After two years of extensive stakeholder engagement and planning, CHANT (Community Access and Navigation in Tennessee) launched in two pilot counties in the summer of 2018. This workshop will present the theory and framework for care coordination for at risk families and the CYSHCN population to address medical needs and social determinants of health while incorporating key Title V performance measures. Initial outcomes and lessons learned will be discussed in the context of pilot evaluation and continuous improvement.
