The expectation for care along the continuum is a key consumer expectation. Assisted living/senior housing will need to seek out partnerships with organizations that are stakeholders in their residents’ care. The challenges are numerous but so are the benefits. This session provides a case study of a successful partnership model that has yielded significant positive health and cost outcomes for residents of Juniper Communities through the Connect 4 Life population health management model.
