Citations for failure to implement abuse prevention policies, failure to investigate, and failure to report are on this rise. This session focuses on the missing elements of an investigation that underlie the majority of the citations. We will approach investigations as a QAPI process using data gathering, root cause analysis, PDSA cycles, and other elements. We’ll introduce several tools that can help you organize and document your investigation and action that are easily adaptable to any adverse event that occurs within your center—reportable or not.

• Discuss the necessity of dedicating the resources and time to a thorough investigation and root cause analysis prior to implementing the final action plan to address an adverse event.
• Identify the most frequent omissions in investigations and gain tools to address these.
• Explain how to incorporate investigations into QAPI.
