Treating the atrophic jaw is becoming more prevalent. Retreatment of previously failing full-arch implants often leaves the patient with no bone without massive grafting. Two-four zygomatic implants on the maxilla can provide fixed solutions often in an immediate fashion. The atrophied mandible often has limited AP spread for a fixed restoration. Relocating the mental foramen distally can provide better AP spread for fixed bridges rather than overdentures. Both of these procedures can be performed in an office setting without residual paresthesia or the need for complex grafting.
