Describe key aspects of the HIV Care Continuum
Describe building a Housing Care Continuum for Broward County
Understand how housing status impacts outcomes along the HIV Care Continuum
Explain best practices and successful approaches for measuring housing related health outcomes.
Discuss strategies for using this data to improve program design and service delivery and ultimately benefit the clients served.
Description : City of Fort Lauderdale is one of the few municipalities that uses an integrated database that tracks both housing (HOPWA) and health care data (Ryan White). An analysis of this data provided support for the positive impact of stable housing on HIV health outcomes and housing’s role in ending the HIV epidemic. The integrated database provides the City with the capacity to track the impact that stable housing has on HIV health outcomes over an extended period. The Housing Care Continuum data positively supports the role that stable housing plays in stemming the HIV epidemic.

Term rent Mortgage Assistance (STRMU), Permanent Housing Placement (PHP) and Housing Case Management on HIV Health Outcomes
Recommendations : Develop a plan to integrate ADAP to compare clients who are HIV positive and are receiving HOPWA, Ryan White and ADAP services.
Develop a plan to annually increase HOPWA participants engagement with Ryan White Part A services by 5% annually.
Develop a plan to identify performance indicators for HOPWA programs.
Develop a plan that ties HOPWA performance indicators to City’s HOPWA measures using Clearpoint Strategy software
