MP30-16 - The diagnostic value of grade of ureteroscopic biopsy on final pathological stage in patients with upper urinary tract urothelial carcinoma

Introduction & Objective :

The objective of this study was to evaluate whether histopathological grade of ureteroscopic biopsy can predict the final pathological stage in patients with upper urinary tract urothelial carcinoma (UTUC).


Methods :

 In retrospective single center study, we evaluate patients with UTUC who were ureteroscopically biopsied before nephroureterectomy (NUx) from Jan/2008 to Apr/2018 at Okayama university hospital. The patients who had neoadjuvant chemotherapy before NUx were excluded. Their clinical and pathologic data were reviewed to determine the positive and negative predictive values of grade of ureteroscopic biopsy to predict the final pathologic disease stage of NUx.


Results :

Seventy-one were included. A definitive diagnosis of UC was made by ureteroscopic biopsy in 68 lesions (95%). Of the urothelial tumors, grade1, 2 or low grade were identified in 26 (38%), grade 3 or high were in 31 (46%), and unknown were in 11(17%). Of the 26 patients with grade 3 or high grade disease, 11 had pT2 disease or higher (42% positive predictive value). Of the 42 patients with less than grade 2 disease, 32 had less than pT2 disease (76% negative predictive value).


Conclusions :

The histopathological grade of diagnostic ureteroscopic biopsy can predict the final pathological stage of NUx in patients with UTUC. The information help identify candidates for neoadjuvant chemotherapy.
