MP19-1 - Evaluation of the Endo-Uro Trainer for Ureteroscopy Training

Introduction & Objective : Reduction in working time hours have limited the use of Operating Room (OR) based surgical training and has resulted in an increased interest in simulation-based training. Several simulator models with different fidelities have been introduced. The aim of the study is to evaluate the face, content and construct validity for the Endo-Uro Trainer (SAMED, Dresden, Germany), a new bench model for Endourology.


Methods : Novices (n=29), intermediates (n=18) and experts (n=20) with a mean age of 30.1 were recruited to participate in the study.
Novices were randomly allocated to Groups A and B using a randomiser tool. All participants received didactic teaching, following which Group A performed a set procedure using the Uro-Scopic Trainer (Limbs and Things, Bristol, UK) and Group B used the SAMED Endo-Uro trainer. 
They were then crossed over and each group performed the same procedure with the other simulator. Participants repeated the procedure twice on each model. The intermediate and expert groups performed the procedure on the Endo-Uro trainer only. A semi-rigid ureteroscopy and mid-ureteric stone basketing was performed by all.
Participants were scored using the Objective Structured Assessment of Technical Skills (OSATS) tool and the timings were analysed. All participants were invited to a final questionnaire, which used a 5-point Likert scale to demonstrate face and content validity.


Results :

Face validation revealed a realism rating in all aspects with mean Likert rating of 4.29/5 (±0.72).
Construct validity in time taken was demonstrated between novices and experts (mean 281.4 secs ± 74.9 vs 204.9 ± 33.2, p=0.0015) as well as between intermediates and experts (mean 245.4s ± 59 vs 204.9 ± 33.2, p=0.0120).
Construct validity was also demonstrated in OSATS rating between all 3 participant groups. Comparison of Group B participants’ performances showed statistically significant improvement in time (373.5s vs 224.3, p=0.0012) and OSATS score (15.64 vs 18.09, p=0.0439) after the cross over.
Group A and Group B post cross over performances showed significant difference in time (p=0.0025) but difference in OSATS was not statistically significant (p=0.0984).


Conclusions : The study demonstrated the face, content and construct validity for the Endo-Uro trainer. However, compared to the well-validated Uro-Scopic trainer, a larger study is required to demonstrate better results using the Endo-Uro trainer.
