VS12-4 - Endometriosis behind the mask of a pelvic tumor spreading to the bladder wall

Introduction & Objective : extragenital endometriosis is a condition that often requires a multidisciplinary approach as endometrial implants, when present in the bladder and ureteral wall, can mimick tumours of these organs. Bladder endometriosis, being a rare condition, is still not fully researched by urologists and gynecologists. Thorough analysis of a patient history, imaging studies and histopathological analysis of specimen obtained during cystoscopy allow clinicians to verify the diagnosis and define adequate treatment strategy.


Methods :

in the video, we present the original case report of a patient with bladder cancer-like presentation alongside the involvement of the right-sided intramural part of the ureter and ipsilateral hydronephrosis. After profound analysis of the patient’s history and clinical studies, the patient was referred to the Gynecology Department, which allowed us to come up with a correct diagnosis and choose an organ preservation surgery as an optimal treatment option.


Results :

the patient underwent a robotic removal of all sights of endometriosis as well as the right adnexa, bladder wall resection and a right-sided ureteroneocystostomy. Control pelvic MR has shown post-operative changes of the right ureteral orifice; no signs of endometrioid hemorrhagic cysts were present.


Conclusions : a multi-disciplinary diagnostic approach allowed us to perform adequate assessment of the patient's history and examination, define optimal treatment option and preserve the bladder.
