MP15-5 - Is it necessary to discontinue anti-platelet drug before robot-assisted laparoscopic radical prostatectomy?

Introduction & Objective : Recently, cardiovascular diseases have been increasing worldwide, and anti-platelet drugs, such as aspirin, have been taken regularly to manage and prevent these diseases. However, these drugs have been considered that they should be stopped before major surgery to prevent risk of bleeding. Discontinuation of aspirin may induce increased risk of signi´Čücant perioperative ischemic events. Although radical prostatectomy is one of the major surgeries, recent development of robot-assisted laparoscopic surgery reduced risk of severe bleeding. Therefore, we compared perioperative outcomes of robot-assisted laparoscopic radical prostatectomy (RALP) in the patients with or without aspirin medication.


Methods : From January 2010 to March 2016, we retrospectively reviewed medical records of the patients who underwent RALP at our institution. We obtained preoperative characteristics such as demographic, medical history, prostate cancer diagnostic details, and D’Amico risk group and perioperative outcomes including operating time, hematocrit change, transfusion, hospital stay, and Clavien complications. The patients were divided into 3 groups according to the aspirin medication; continuation of aspirin (group 1), preoperative discontinuation of aspirin (group 2), and no aspirin medication (group 3).


Results : The number of patients in group1, 2 and 3 were identified as 15, 49 and 266, respectively. Although the proportion of high risk and advanced stage patients were significantly higher in group 1, operating time, postoperative transfusion rate, duration of hospital stay, complication rates and hematocrit change were not significantly different among the three groups.


Conclusions : This study demonstrated that peri-operative continuation of aspirin did not deteriorate surgical outcomes of RALP.
