VS9-3 - Establish Extra-peritoneal Space for Robotic-assisted Radical Prostatectomy and Lymphadenectomy

Introduction & Objective : Extra-peritoneal robotic-assisted radical prostatectomy (RaRP) with lymphadenectomy have showed benefit on reduce bowel complications and avoid prior abdominal surgical scars. However, the extra-peritoneal space is more difficult to establish. This video is aim to share our experience and how to create surgical space for this surgery and performe lymphadectomy. 


Methods : There were total 35 patients underwent RaRP by a single surgeon (Victor C. Lin) between May, 2016 and May, 2017. 24 patients of this cohort underwent extra-peritoneal route operation. The characteristics and peri-operative outcomes were collected retrospectively. The procedure detail is showed in the video. 


Results : The mean age was 67.6 and the mean pre-op PSA was 11.1 ng/ml. There were 71% patient has cT2 disease and the rest were cT1. The mean port setup, lyadenectomy and console time was 28, 21 and 144 minutes, respectively. The patients had oral diet on the mean 1.4 days. The mean lymph node yield was 19. There was no grade 3 or 4 complication.


Conclusions : The extra-peritoneal RaRP is feasible and can provide good outcomes and this procedure showed in the video can be duplicated.
