VS8-1 - Pulsating Organ Perfusion System in the Training of Robotic Partial Nephrectomy

Introduction & Objective : Robot assisted partial nephrectomy (RAPN) is a challenging procedure that requires both advanced excision and reconstruction skills. Training models which are very similar to that in human anatomy would be very beneficial to shorten learning curve for this procedure. Feasibility of the pulsating organ perfusion system (POP) in the training of RAPN was presented in this video.

Methods : Fresh frozen porcine kidneys with their intact aorta and vena cava were used for training of RAPN in the POP trainer system. Abdominal aorta was catheterized and connected to the pump outlet of the POP simulation trainer. Tap water with red dye was used as a perfusion medium simulating blood. Cava and ureter were left opened. Porcine kidneys are supported on a perforated metal grid. Camera, 1st and 2nd arm ports could be placed through the neoprene anterior wall but the 4th arm had to be placed through lateral outlet of POP trainer.

Results : The POP trainer system successfully enabled to perform all of the RAPN steps; namely dissection of the hilum, placement of Bulldog clamps, excision of the predefined renal parenchyma, control of the bleeding vessels by suturing and placement of renorraphy sutures, respectively.

Conclusions : It seems feasible to simulate RAPN on POP system. Dedicated training on the POP system may shorten the learning curve of RAPN.
