Introduction & Objective : Cyroablative therapy for small renal masses is associated with higher local recurrence, and is typically managed with radical nephrectomy. In well selected patients that would benefit from nephron sparing, we performed feasilbility of partial nephrectomy for managment of a larger (cT1b) renal mass after a cyroablative treatment. 

Methods : Patient is a 64 year old female with CKD 3A (eGFR = 55) with history of nephrolithasis and general abdominal pain, that was managed with percutaneous cryoablation of a 3.4 cm mass. She then had a local recurrence with >6cm exophytic mass of the lower pole of the right kidney. 

Results : We performed a robotic assisted right partial nephrectomy. Total ischemia time of 41 minutes and EBL of 75 cc. She had an uneventful recovery and was discharged home without complication. Renaul function was unchanged at time of discharge. 

Conclusions : Partial nephrectomy is feasilble in select post cryoablative recurrence even in the setting of a large renal mass.
