VS4-4 - Modified endoscopic technique for management of Intrauterine Device migration into the bladder

Introduction & Objective :

The treatment options for an Intrauterine device (IUD) embedded into the bladder are wide ranging from endoscopic procedures to open or laparoscopic surgeries. In this study, we report a case of ectopic intravesical IUD whom underwent a new modified successful endoscopic IUD removal.

Methods :

A healthy 37-year-old woman referred to our clinic with one year history of recurrent urinary tract infection (UTI), dysuria and frequency. Her past medical history informed us that an IUD (Copper TCu380A) had been inserted 11 years ago. A plain abdominal radiograph was requested which showed a metallic foreign body in the pelvis.

Results : We tried to remove the IUD by cystoscopic forceps but it had strongly invaded into the bladder wall. Subsequently, we made a superficial cut in the bladder mucosa and muscle with J-hook monopolar electrocautery and extracted it completely with gentle traction. The patient was discharged one day after IUD removal without any complications.

Conclusions : IUD migration into adjacent organs may remain asymptomatic for a long time and every woman should be informed about such serious complications before an IUD insertion procedure. Our modified cystoscopic extraction technique is recommended as a safe and useful method for extraction of partially intravesical IUDs.
