MP4-4 - Does it matter who perform multiparametric magnetic resonance imaging directed cognitive prostate biopsy to detect prostate cancer? The comparison of urologists and radiologists

Introduction & Objective : This study aims to compare the performance of urologists and radiologists on the use of multiparametric prostate magnetic resonance imaging (mp-MRI) directed cognitive systematic transrectal prostate biopsy (CSTPBx).

Methods : Between January 2017-January 2018, 349 mp-MRIs were performed at the department of radiology. One hundred and one patients who underwent mp-MRI directed CSTPBx for PIRADSv2 score 3, 4 and 5 lesions were included into this retrospective study. The patients were divided into two groups according to whether the biopsy was performed by urologist (Group 1, n=41) or radiologist (Group 2, n=60).  Categoric and non-parametric continous variables were compared between two groups using chi-square and Mann-Whitney U tests, respectively.

Results : The patient age, clinical T stage, PSA, PSA density, prostate volume were similar in two groups. The proportion of PIRADSv2 score 3, 4 and 5 lesions were not statistically different between two groups. However, the relative increases of PIRADS 3 lesion in Group 1 (34.1 vs 15.0 %) and PIRADS 4 lesion in Group 2 (51.2 vs. 70.0 %) were notable. The pathology report did not significantly differ between two groups meanwhile the number of total cores were significantly higher in Group 1 (12 vs. 11, p=0.007). The proportion of Gleason Grade and positive core numbers in prostate cancer patients were not different between two groups. 

Conclusions :

This retrospective study showed that the performance of urologists and radiologists were similar on mp-MRI directed CSTPBx for Pca detection. However, the prospective studies with extended cohorts are needed to provide further suggestions.
