VS3-3 - Tips and Tricks of Holmium Laser Enucleation of the Prostate (HoLEP)

Introduction & Objective : Holmium laser enucleation of the prostate (HoLEP) is the robust and durable surgical option for the management of lower urinary tract symptoms (LUTS) secondary to benign prostate hyperplasia (BPH) independent of gland size. The steep learning curve for HoLEP has previously been reported as a hindrance for wide adoption of this surgical technique. From a high volume experience at our institution, several nuances of HoLEP have been determined and herein these are demonstrated to help improve understanding of this difficult procedure. 


Methods : Between 1998 and 2017 almost 4000 HoLEPs were performed at our institution. A subset of cases were reviewed between these dates and selected for video demonstration of interesting teaching points. 


Results : All patients underwent the HoLEP procedure at IU Methodist Hospital. Video footage was identified to demonstrate valuable teaching points for surgeons performing HoLEP. The topics included in this video abstract include: one cut versus two cut initial groove, concomitant cystolithalopaxy, HoLEP after Urolift, subtrigonal enucleation, HoLEP for prostatic abscess and HoLEP in patients on anticoagulation.


Conclusions :

HoLEP is a very versatile surgical technique and we present tips and tricks to aid other surgeons with this procedure.
