MP19-9 - A Structured Model for training Transurethral Resection of prostate and bladder tumour (TUR-P/BT) at an Intensive Urology Simulation Boot camp: Trainee’s feedback

Introduction & Objective :

There has been a shift in attitudes towards surgical training with an emphasis on demonstrating a level of competency prior to operative exposure. In addition, the European Working Time Directive and advances in medical therapy have reduced the numbers of TUR-P/BT being performed. Simulation based teaching is a widely accepted training method and is being used to help hone trainees’ skills in TURs. An intensive 5-day Urology Simulation Boot-camp was run in Leeds to teach junior trainees technical and non-technical skills, including TUR-P/BT.

This study aims to report the feedback from candidates and explore the suitability of such a programme to be delivered as part of the training curriculum.

Methods : In 2017, 42 UK urology trainees took part in the boot camp. The module had six stations and participants spent an intensive 4hr period learning about the equipment and developing their operative skills on using SAMED tissue models and TURMentor Virtual Reality (VR) models. Filling an Ellick evacuator and performing a bladder washout to clear clots was also covered. Faculty to candidate ratio was 1:1. Feedback on course structure and learning points was obtained upon completion, with ratings from 1 (low) to 5 (high).

Results : Qualitative and quantitative feedback was obtained as in Table 1:

Overall the feedback was positive, receiving a mean score of 4.33. Teaching on the resectoscope equipment was highly rated, mean score 4.7, highlighting the importance of covering the fundamentals. The SAMED model for TURP received praise (mean rating 4.5) but occasional systems failures negatively impacted VR model feedback (mean rating 4.1). These were resolved for TURBT exercises. Conclusions : Our structured and systematic training model incorporating cognitive knowledge with practicing basic steps of the procedure and dealing with complications was well received with improvement in trainee’s knowledge and confidence. The results support a high level of face-validity for a multi-component teaching curriculum for TUR training.
