Implementing a Transition Council to Achieve Employment Outcomes

The presenters have over 20 years of collaborative work together and have developed a variety of work based learning options that move the classroom to places like hospitals, colleges and local businesses. As the pressure to find employment outcomes for individuals with I/DD increases, a strategy that brings together education, vocational rehabilitation, business and organizations that support people with I/DD is critical in assuring students are afforded employment options based on the principles of employment first.

The Carroll County Transition Council provides a holistic, best practice approach to planning and implementing effective transition plans for students. The Transition Council is a dynamic group that is charged with developing a multi focused process that considers the key aspects to effective transition planning. The core components and sub groups of the Council are focused on: collaboration with over 25 service providers and businesses, adult I/DD providers and behavior health services, transition fair for students and families, transportation, communication, youth connections, employment and digital student portfolio. Plans will be shared on the newest components which include; family engagement, self determination, IEP transition planning and CTE-CRD completer. The collaboration between The Arc and Carroll County Public Schools is transformative as The Arc becomes engaged with students at age 14 and is involved in planning and providing contracted services prior to the student exiting. The result is a comprehensive plan that considers the transitioning student’s career/employment interests, transportation needs, social needs, residential needs and the types of personal support they may require to be successful. The program also includes opportunities for work based learning that is funded by the Division of Rehabilitation Services and another important Council member. This presentation is for organizations that are involved in transition planning who want to improve outcomes, organizations that want an additional funding/contracting opportunity and organizations that want to increase their compliance with the Community Settings Rule.

Participants will be able to take away a best practice process for implementing a seamless transition program that benefits students and their families during a critical phase of their development. The presentation includes handouts, clear examples of successful transition plans that can be replicated, and identifying stakeholders who are important to the process.

Donald Rowe

Executive Director
The Arc Carroll County

Donald Rowe is the executive director at The Arc Carroll County in Westminster, Maryland. He has an MBA from Frostburg and a Masters in Special Education from McDaniel College. He obtained a B.S. in Social Work from Elizabethtown College. He has worked in the area of developmental disabilities for over 30 years. Don has assisted individuals in starting their own businesses and transitioned his organization from a workshop to an employment services model Don currently serves on Maryland's Tiered Standards work group and was appointed to serve Maryland's Department of Disabilities board, along with serving as a director for Maryland Association of Community Services. Don is also a program and admin surveyor for CARF.

Mary Pat Dye

Program Coordinator of Post Secondary Services
Carroll County Public Schools

Mary Pat Dye received her Master of Science in Special Education from Johns Hopkins University in 1995. At present time, she is the Program Coordinator of Postsecondary Services and Transition Services for Carroll County Public Schools. Her primary role is to develop and implement postsecondary programs for certificate bound students age 18-21 as well as provide consultative services and technical assistance in the area of student transition planning and transition service delivery.
