Lessons learned from the WCRF/AICR Expert Reports on Diet, Nutrition, Physical Activity and the Prevention of Cancer: Implications for the Future of Nutrition and Cancer Research

Since 1997, the World Cancer Research Fund (WCRF) International and the American Institute for Cancer Research (AICR) have been at the forefront of synthesizing and interpreting the accumulated scientific literature on the link between diet, nutrition, physical activity and cancer, and deriving evidence–based Cancer Prevention Recommendations. The 2007 WCRF/AICR 2nd Expert Report, using a robust method and systematic literature reviews, was a landmark in the analysis of evidence linking diet, body weight and physical activity to cancer and led to the establishment of the Continuous Update Project (CUP). New findings from the CUP have been released for 12 cancers, and updated findings for colorectal, lung and mouth/larynx/pharynx will be released in late 2017.The 3rd WCRF/AICR Expert Report will be published in January 2018 and will include a new synthesis of the current evidence from the CUP and an update of the WCRF/AICR Cancer Prevention Recommendations. The purpose of this symposium is to present the latest evidence on the role of diet, nutrition and physical activity for cancer prevention; to discuss how the evolution of robust systematic reviews supports the process of interpreting the evidence; and to outline how these findings inform future directions for nutrition and cancer research.

Dr. Marian L Neuhouser is a Full Member in the Cancer Prevention Program, Division of Public Health Sciences at the Fred Hutchinson Cancer Research Center in Seattle, Washington as well as Core Faculty in Nutritional Sciences and Affiliate Professor of Epidemiology at the University of Washington. Her research focus for the past 20 years has been the role of nutrition in the prevention of breast and prostate cancers and other diet-related chronic diseases. She is Principal Investigator or Co-Investigator of multiple NIH-funded grants ranging from controlled feeding trials to large population-based cohorts, including some based on special populations such as minorities and the elderly. Dr. Neuhouser also leads the Fred Hutch’s Nutrition Assessment Shared Resource and spends a focused portion of her research on improving dietary assessment methods. Dr. Neuhouser has authored over 250 peer-reviewed publications and a dozen book chapters related to dietary assessment methods, diet and health and cancer prevention. Dr. Neuhouser is a member of the American Association for Cancer Research, the Academy of Nutrition and Dietetics, the American Society for Preventive Oncology and the American Society for Nutrition. She is currently Vice-President of the American Society for Nutrition and holds other leadership positions within the Women’s Health Initiative and the Southwest Oncology Group (SWOG). Dr. Neuhouser served as a member of the 2015 DGAC.

Steven K. Clinton, MD, PhD, is the Robert A. and Martha O. Schoenlaub Cancer Research Chair at OSU Comprehensive Cancer Center and a Professor of Internal Medicine in the Division of Medical Oncology at The Ohio State University. Dr. Clinton’s training for his MD and PhD at the University of Illinois in Urban-Champaign was followed by an Internal Medicine internship and residency at the University of Chicago. He proceeded with Medical Oncology training at the Dana-Farber Cancer Institute and Harvard Medical School, where he remained on faculty for nearly a decade.

Dr. Clinton joined The Ohio State University in 1998. As a clinician, he specializes in prostate cancer, which is the most common malignancy in American men. Because the course and progression of prostate cancer is profoundly heterogeneous, wise and integrative strategies for optimal care are essential. Dr. Clinton is recognized by US News and World Reports and other agencies as one of America’s “Best Doctors” and serves as Director of the Prostate and Genitourinary Oncology Program at The James Cancer Hospital and Solove Research Institute. Under his leadership, the clinical program has contributed significantly to advances in care as a national leader in accrual to therapeutic and prevention clinical trials. The Prostate Cancer effort is particularly renowned for an integrated multidisciplinary clinic (medical oncology, radiation oncology, and urology) which guides patients with newly diagnosed prostate cancer towards optimal care plans uniquely tailored to each individual.

Dr. Clinton serves as the Program Leader for Molecular Carcinogenesis and Chemoprevention (MCC) at The Ohio State University Comprehensive Cancer Center (OSUCCC). This program is one of five at the OSUCCC and added greatly to the recent successful review of the cancer center by the National Cancer Institute in 2015, with the remarkable achievement of a perfect score of “10.” The focus of the MCC program, involving over 40 faculty, is elucidating the fundamental mechanisms underlying the development of cancer and defining prevention strategies. A major strength of the program is the “crops to the clinic” research agenda that integrates the efforts of scientists in agriculture, food science, and nutrition with clinical investigators to conduct novel human clinical trials at The James. This effort is unique among the nation’s 45 comprehensive cancer centers, and these teams have published novel research focusing upon the roles of nutrients, such as Vitamin D and omega-three fatty acids for cancer prevention, and have also pioneered the development of novel food products based upon various whole foods, including soy, tomatoes, and black raspberries for cancer prevention.

Dr. Clinton is very active in bionutrition research at OSU. He serves as Associate Director for the campus-wide Center for Advanced Functional Foods Research (CAFFRE) and the OSU Food Innovation Center (FIC), programs dedicated to the development of novel food products and their evaluation for promoting health outcomes and improving global nutrition. Most recently, he is serving the University’s Discovery Theme effort to build a campus-wide program in Nutritional Metabolomics for Human Health.

Dr. Clinton provides service to many national and international organizations involved in the war on cancer. He served as the Chairman of the 25th Anniversary Meeting of the American Institute for Cancer Research / World Cancer Research Fund, entitled “Celebrating a Generation of Cancer Research,” which was held in Washington DC in November 2016. He is currently serving on the World Cancer Research Foundation Advisory Committee to define cancer prevention strategies involving diet and nutrition, with their reports being utilized by the World Health organization and many nations around the globe for public health initiatives, as well as agricultural and food policy. Dr. Clinton has recently completed service to the United States Department of Health and Human Services and United States Department of Agriculture to prepare the Dietary Guidelines for America 2015-2020 Report, which impacts hundreds of food and agriculture programs in the United States. He has previously served the Institute of Medicine/National Academy of Sciences to define Dietary Reference Intakes for Calcium and vitamin D.

Dr. Clinton’s research activities at OSU focus on many aspects of diet, nutrition, and cancer, primarily emphasizing prostate and other genitourinary cancers. The research efforts include metabolic epidemiology and clinical intervention trials, as well as basic laboratory studies of cellular and molecular biology resulting in over 250 scientific publications, reviews, and book chapters.

Dr. Stephen Hursting is Professor in the Department of Nutrition at the University of North Carolina (UNC) at Chapel Hill. He is also Professor at the UNC Nutrition Research Institute and a member of the UNC Lineberger Comprehensive Cancer Center. He earned his PhD in nutritional biochemistry and MPH in nutritional epidemiology from UNC-Chapel Hill, and he completed postdoctoral training in molecular carcinogenesis and cancer prevention at the National Cancer Institute (NCI). Prior to joining the UNC faculty in 2014, Dr. Hursting was Professor and Chair of the Department of Nutritional Sciences at the University of Texas (UT) at Austin, the McKean-Love Endowed Chair of Nutritional, Molecular and Cellular Sciences in the UT College of Natural Sciences, and Professor of Molecular Carcinogenesis at the UT-MD Anderson Cancer Center (2005-14). He also previously served as Chief of the NCI’s Nutrition and Molecular Carcinogenesis Laboratory Section (2000-2005) and Deputy Director of the NCI’s Cancer Prevention Fellowship Program (1999-2005). His research interests center on precision nutrition as applied to cancer prevention, particularly the molecular and metabolic mechanisms underlying obesity- cancer associations, and the interplay between obesity, metabolism and cancer. Primarily using preclinical models (including human and mouse cell lines, genetically engineered mouse models of cancer, and genetically heterogeneous Collaborative Cross mice) in parallel with human studies, he is currently focusing on the molecular and metabolic changes occurring in response to lifestyle-based (dietary and physical activity); surgical (bariatric surgery), or pharmacologic manipulation of energy metabolism and cell signaling pathways, with emphasis on growth factor signaling pathways, inflammation and the gut microbiome.
