E5 - Using Data to Transform Community and Clinical Practice for Better Integrated Mental and Physical Healthcare

Wednesday, July 11
1:30 PM - 2:00 PM
Location: Salon F\G

This session will demonstrate the value of coupling mental health data from Electronic Health Records (EHRs) with traditional surveillance data. Pairing this data can provide physical and behavioral healthcare providers with neighborhood-level profiles to illustrate action steps that can be taken within the clinic setting and in partnership with communities to more effectively plan for and address the mental health needs of communities. The session will also illustrate how mapping social determinants of health data and clinical diagnoses expands our understanding of the complexities involved in the prevention and treatment of mental health issues.
