Differentiation of Clinical Intervention in the Treatment of Persons with Dissociative Identity Disorder Presenting with Erotic vs Eroticized Transferences

Erotic transference creates a disturbance in the therapeutic field discomforting patients and therapists. Therapists are troubled about how to traverse this territory. Eroticized transference is even more problematic. Erotic transference is an arena for a multiplicity of errors. Eroticized transference is potentially the arena of therapeutic stalemate or disaster. Survivors of sexual abuse present for psychotherapy with relational difficulties they are unable to source.  Unconscious, dissociated experience is presented through implicit communication conveyed in the intersubjective field transferentially. Confusion and terror predominate.  Love and sex become merged. Often in their families of origin there is no differentiation between feeling and behavior, between desire and action. Love and affection are interwined with betrayal and sexual intrusion. Kindness evokes fear. The desire for care and connection and the need for attachment risks exploitation and abandonment. Patients who are survivors of sexual abuse are more likely to have sex with their therapists. Therapists who are survivors of sexual abuse are more likely to have sex with their patients. Therapists often become rigid, avoidant, aggressive and/or rejecting, fulfilling survivors negative relational expectations. Avoidance and neglect of survivors’ therapeutic needs are as problematic as premature intrusive interpretations.
