Dissociation in Obsessive Compulsive Disorder: Trauma-Related Disturbances of Sense of Self-and Agency

This study is concerned with relationships between childhood trauma history, dissociation, and clinical phenomenology of obsessive compulsive disorder. Fourty-two patients with  obsessive compulsive disorder consecutively admitted to the outpatient unit of the Cerrahpasa Medical Faculty Hospital (Istanbul) were evaluated using Structured Clinical Interview for DSM-IV,  Yale-Brown Obsessive Compulsive Scale, Dissociative Experiences Scale, Dissociative Disorders Interview Schedule, Childhood Trauma Questionnaire, Spielberger Anger Scale, and Beck Cognitive Insight Scale. Mean age of the patients was 30.3 (SD=9.1). Four-teen (33.3%) patients fulfilled criteria of a dissociative disorder according to the DDIS concurrently. Eight patients had dissociative disorder not otherwise specified, 4 patients had dissociative amnesia, and 2 patients had depersonalization disorder. None of the patients had dissociative identity disorder or dissociative fugue. There were significant correlations between childhood trauma and DES but not YBOCS scores. DES scores were also correlated with trait anger and anger inside. There was negative correlation between DES scores and anger control. Childhood trauma scores were correlated with anger inside but not with anger control, trait anger, or expressed anger. In the path analysis model, dissociative, supranatural, and passive influence (Schneiderian) experiences were mediators between childhood trauma and both obsession and compulsion scores.

Due to his extensive studies on this area, Prof. Sar received David Caul Memorial, Morton Prince Scientific Achievement, Cornelia Wilbur, Pierre Janet Writing, and Lifetime Achievement Awards of ISSTD. He sits in the editorial boards of the European Journal of Psychotraumatology and the Journal of Trauma and Dissociation. Vedat Sar is President to the European Society for Traumatic Stress Studies (2013-2015) and former President of the International Society for the Study of Trauma and Dissociation (2008-2009).

In 2015, I worked with traumatized clients diagnosed with substance and alcohol dependence in both individual and group psychotherapy settings at Solid Landings Behavioral Health. In the meantime, I worked at Diamond Family Counseling, where I practiced Emotionally Focused Couples Therapy with couples. My psychotherapy trainings include Eye Movement Desensitization and Reprocessing EMDR I – II (Gerald Puk, Ph.D.), Parent-Child Interaction Therapy PCIT (Carrie Axline-White, Ph.D.), Emotionally Focused Couples Therapy, EFT Externship (Sue Johnson, Ph.D.), Compassion Focused Therapy (Paul Gilbert, Ph.D.), Erickson Hypnotherapy Masterclass & Intensives (Jeffrey Zeig, Ph.D.).I have been working as a psychotherapist at Koç University Hospital in Turkey since 2016, where I had the chance to meet Professor Sar and I am willing to do research on the etiology of trauma and dissociation and also hopefully on psychotherapy outcomes.
