235. Home Care: Best Practices and Future Implications for Our Profession

As citizens age, it is imperative that caregivers monitor nutrition and implement strategies to assist with graceful aging and quality of life. Since 1972, the Department of Veterans Affairs (VA) has operated an innovative healthcare program that brings medical services to veterans at home. This program has resulted in cost-savings and quality of life improvements of these sick patients, who average 19 clinical diagnoses and 15 medications. Patient care has improved in a number of areas, such as outcomes, satisfaction, and coordination. The dual purpose of the session would be to focus on the impact of the RDN on an interdisciplinary care team as well as to discuss the strategies or goals to assist the elderly in maintaining as high a quality of life as possible. Case studies will be discussed and attendees will leave feeling inspired to work with geriatric clients, and to begin to consider providing home nutritional services either for work or as an entrepreneur.

Angel Planells, MS, RDN, CD

Angel Planells works as lead dietitian in the home-based primary care program at the Veterans Health Administration in Seattle, Washington. He is also owner of a private practice and consulting firm, ACP Nutrition. Planells is currently a National Media Spokesperson and Fellow of the Academy of Nutrition & Dietetics, President of the Washington State Academy of Nutrition & Dietetics (WSAND), Past-President of the Greater Seattle Dietetic Association (GSDA). He has been honored for his work as chair of the Academy's National Organization of Men in Nutrition member interest group for recruiting and retaining men in the dietetics profession. Planells holds a certificate in adult weight management. He is a graduate of Tulane University and earned a master's degree from Columbia University.

Cassandra Holt, RDN

Cassandra is a Licensed Clinical Registered Dietitian Nutritionist for the Veteran’s Health Administration, Fort Worth, TX. Her practice involves Home Based Primary Care, for geriatric veterans who are home bound. She is committed to providing the current best practices and evidenced based care to the veterans she follows. She provides nutrition education to Medical Foster Home caregivers at the bi-annual caregiver’s conferences, educating the caregivers on safe food practices in their homes. She is the 2017-2018 Academy of Nutrition and Dietetics Diversity Leader, a 2017 Leadership, Effectiveness, Accountability, Development graduate at the Veteran’s Administration and holds a certificate as a Yellow Belt in Lean Six Sigma Methodologies.

Katherine Dodd, MS, RDN, CSG, LD, FAND

Katie M. Dodd is a Registered Dietitian Nutritionist, Board Certified Specailist in Gerontological Nutrition (CSG), and Fellow of the Academy of Nutrition and Dietetics. She lives in southern Oregon and works for the Veteran's Health Administration. Katie has more than 10 years experience as a Home Based Primary Care dietitian. Katie is the 2018/19 Chair of the Healthy Aging DPG, a past President of the Oregon Academy, and a CDR Specialty Examination Workgroup member invovled in CSG exam development. Katie periodically instructs webinars for Dietitian Centeral and is a content editor and reviewer for Nutrition Dimensions/On Course Learning.
