As one of two quality management specialists for the care coordination program, Kristecia Estem provides technical assistance to organizations in New York City receiving Ryan White Part A-funding to provide care coordination program services. Ms Estem conducts site visits to organizations implementing the intervention, facilitates meetings of care coordination program providers, and collaborates in the design and delivery of trainings for care coordination program staff. Ms Estem joined DOHMH in 2015 and is responsible for supporting 17 care coordination programs with capacity-building assistance. She regularly provides guidance to program staff addressing service definitions, guidelines for documenting and reporting services and intervention implementation.
Ms Estem coordinates the development and implementation of evidence-informed guidelines to assess and promote clients’ HIV care self-management skills, a new component of the intervention. She consults with key stakeholders in the design and delivery of self-management assessments and collaborates in the development of training protocols for care coordination program staff.
