Session Description
Patient centered healthcare has emerged out of the movement to improve patient outcomes and satisfaction while saving costs, time and resources. The healthcare provider that practices patient centered care recognizes it is not the number of tests ordered or procedures performed but the quality of the partnership between the patient and the clinician that drives patient satisfaction which often enhances outcomes through increased patient engagement in their care. This program will focus on the growing number of supporters seeking blended learning designs that address the science of disease and evidence-based practices, as well patient satisfaction and engagement. This program will summarize a comprehensive review of patient centered grant supporters, disease state areas and their selection/evaluation criteria. Furthermore, the 2017 White House Innovation Award winning patient oncology education online application Prescription-To-Learn (P2L) will be demonstrated through exploration of curated resources, patient and physician analytics.
