Longer Dwells; The PIV Enigma

With a better understanding of vein depth and catheter length, the rapid explosion of ultrasound guided PIV insertions have begun to yield longer dwell times for peripheral catheters (PIV). Better products, equipment and better techniques have helped some institutions decrease their PIV restarts by increasing dwell time to 5 days or more on 35% of their patients.
What's the difference? Is there something that clinicians are missing? Does the way the catheter lays in the vessel affect the success of the infusions? And as the duration of PIV catheter dwell times increase and the move to clinical indication increases, how can the clinician minimize the risk of complications by the choices of catheter and particularly the standardization of insertion techniques and ensuring they are all done in an aseptic manner. Why treat PIVs any differently than other CVADs?
As we look inside of the vein more closely, we begin to see that we do not truly understand what is happening.
