C1 — Catheter to Vein Ratio and Its Impact on Vein Preservation

We have very limited knowledge of the natural history of veins that have been used for IV access. Experience suggests that patients with multiple peripheral venous catheters undergo venous depletion requiring more invasive lines to provide for access needs. Many factors may influence venous depletion. Many currently believe that a high ratio of catheter size to vein diameter is a significant contributor to this problem. How do we assess this and what is the current data? How should we judge the appropriate catheter size for a given vein? How do we fight against the entrenched bias of bigger is better? This presentation is review our current knowledge, provide some new information related to this issue and suggest what best practice should be based on that information. Hopefully with this information we can impact the issue of venous depletion and the creation of difficult venous access patients.

Dr. Gregory J Schears, MD is a physician working at the Mayo Clinic, Rochester, MN. He received his medical degree from the University of Wisconsin School of Medicine and Public Health. He completed his Pediatric residency at St. Louis Children's Hospital, St. Louis MO. his Anesthesiology residency, Pediatric Anesthesiology fellowship and Pediatric Critical Care fellowship at Johns Hopkins Hospital, Baltimore, MD. He was recruited to Mayo Clinic in 1991 from the Children's Hospital of Philadenphia to start their ECMO program and participate in the Congential Heart ICU and multi-specialty ORs. He has an over 30 year interest in understanding and improving our approach to vascular access. He has given hundreds of lectures locally, nationally and internationally and helps to lead the nurse PICC team at Mayo Clinic.
